[Bug] Thrown bombs sometimes do not work/are blanks?

Observed behaviour

When I throw any kind of bomb there is a 1-in-20 ish chance that one do not work. They are thrown but don't do anything upon landing or can't be seen landing at all.

Expected behaviour

Bombs work as usual

Steps to reproduce

Go in creative and start spam throwing bombs.

Other mods

none. but I think using other mods somehow increases the chance of one of the thrown bombs being blanks?
