Distortion noise on new install

Ive bought a pihat and set it up today (pre built pihat from misterfpga.co.uk)

It all looks like its working okay, except I get this terrible noise when i try to run something like monkey island.

I think Ive followed the instructions to a tee, added the lines for the pi-hat in the config, the mister recognises it, i can hear sound behind the distortion.

The noise is the same if using the digital out or hdmi.

Can anyone offer any advice on where im going wrong?
