Make anonymization of survey results optional.

By default, the application should export with phone number. The moderator should have an option to anonymize the results. This is set on survey level.

@danielfdsilva added a hotfix that makes anonymization when exporting the survey results optional.

Still need to be able to properly anonymize data.
