You might want to look at the columns the voteatx team chose for polling locations when that project was active. https://github.com/open-austin/voteatx-svc/blob/master/data/co.travis.voting-places/20120529/Voting_Places.csv

You've probably already considered matching the Google Civic Information API
format. Looks like voteatx geocoded the addresses themselves and included that information in the API, while Google doesn't.
