Changes from additional work during masters thesis to:

Add some additional segmentation evaluation functions to the segmentation module
Resolve the color encoding error by only using the first color channel
Add details of the dicom colour scheme to databases (to support error checking)
Removed a number of unnecessary function incorrectly left in d04_segmentation/segment_utils.py
